Case Management / Social Work Services
Registered Nurse Case Managers and Social Workers at Trinity Medical Center facilitate a teamwork approach during your hospital stay to help coordinate your care. They will work closely with your physicians, nurses, other members of your healthcare team, insurance companies and community agencies to coordinate a plan of care that is in the best interest of you, your family and the hospital. The staff will assist you in your post hospital discharge needs by arranging for home health care services, durable medical equipment, skilled nursing facility placements and rehabilitation referrals. Department hours are from 8:00 a.m. to 4:30 p.m., Monday through Friday, with a worker on-call after hours and weekends to address your needs.
