When you leave the hospital, you may have special needs. Discharge planning is a service which ensures that you have a planned program for continuing care upon leaving the hospital. Through the use of this program, you may receive information that will allow you to take better care of yourself. Case managers, social workers and assistants, in collaboration with your physician and nurses, assist you and your family in identifying your needs and the resources to meet them. Our Case Management professionals are available from 8 a.m. to 4:30 p.m., Monday through Friday to discuss your options for care after discharge.

Transitional Care Facility ext. 1050
Located on the tenth floor of the main hospital, this unit provides care for those who no longer need acute hospital care, but still require short term skilled nursing care. For more information, call 205-592-1050.
